For your convenience, the following forms are provided to open a new account or modify your existing account. The Member Service Request form should be completed to become a member and brought to any KSCFCU branch. Any other form may be completed by current accountholders and returned to the Credit Union at the address below. All forms that require a signature must be notarized.
